1.  Medicare - This is the link that has information about Medicare Part A, Part B and Part D.  This site also has information about nursing homes and their surveys.

2. Ombudsman program:  This is a resident advocacy program for seniors and residents of long term care facilities.

3. Illinois Department of Public Health:  This is the State regulatory agency that licenses our facility for Medicaid and certifies for Medicare.  You may also find statutory forms including Power of Attorney, Living Will and other helpful tools.
